He frames it as the 3A model: affordability, access, and availability. These are not aspirational ideals. If the constraints of the end market are built into the design brief from the very beginning, and not retrofitted as cost-reduction exercises after the fact, the resulting product will be fundamentally different from one designed for the top of the market and then adapted downward. The assumption embedded in this approach is that the market’s needs are stable enough to justify it. Gaurav challenges this directly. Acknowledge the import dependencies that matter most – the ones where domestic capability would have the greatest clinical and economic impact. And champion those efforts through public procurement, because aspiring to self-reliance while continuing to buy imported equipment is a contradiction that undermines the very ecosystem you are trying to create.

The early results of this approach are visible. Initiatives like the Andhra Pradesh MedTech Zone, which is now among the world’s largest MedTech ecosystems, are producing CT scanners, MRIs, Cath labs and linear accelerators under one roof, many of them export-ready. India’s MedTech industry, valued at approximately $5 billion before the pandemic, is projected to reach $30 billion by 2030. The government’s Viksit Bharat framework aims to make India one of the top three global medtech markets. The trajectory is clear, what remains is the discipline to follow through.

From Volume to Value, and From Local to Global

The thread running through this conversation is a single, demanding idea: that the most meaningful innovations are those that close the gap between what is technically possible and what is universally accessible.
